IRDAI Information and Cyber Security Assurance Audit
Codec Networks is an Information Security Auditing Organization, providing independent, evidence-based and risk-focused audits to help insurers, insurance intermediaries and other regulated entities covered under the applicable IRDAI Information and Cyber Security Guidelines, 2026. The audit evaluates the design and operating effectiveness of applicable controls covering cybersecurity governance, risk management, asset security, identity and access management, data protection, application and infrastructure security, cloud services, security monitoring, incident response, business continuity, third-party security and regulatory compliance.
Codec Networks is a CERT-In empanelled auditing organization mandated to offer this service.
Each engagement is conducted against an agreed and management-approved scope, considering the auditee’s regulatory classification, critical assets, technology environment and applicable controls within the prescribed 348-control audit checklist. Codec Networks applies an evidence-driven methodology comprising document and record review, stakeholder interviews, configuration and log validation, representative sampling, authorized technical testing, and assessment of control design and operating effectiveness. Identified gaps are supported by verifiable evidence, risk-rated and communicated with practical remediation recommendations and defined closure requirements.
The engagement may deliver an executive summary, applicability and compliance assessment, detailed risk-rated findings, evidence references, management action plan, audit certificate and remediation-revalidation report, as applicable. The service represents an independent, point-in-time assessment based on the agreed scope and evidence made available; it does not constitute IRDAI or CERT-In certification, endorsement or guaranteed regulatory acceptance, and it does not guarantee the identification of every vulnerability. Responsibility for compliance, remediation and continued operation of controls remains with the auditee organization.
IRDAI Information and Cyber Security Assurance Audit
INDUSTRY SIGNIFICANCE AND SERVICE RELEVANCE
Why this service matters to your industry, where it fits in your security
programme, and what your organisation gains from it.
Insurance organizations increasingly depend on interconnected digital platforms, cloud services, intermediaries, outsourced service providers and customer-facing applications to deliver critical services and process sensitive policyholder information. Cyber incidents affecting these environments can lead to operational disruption, financial loss, data exposure, customer harm, regulatory scrutiny and reputational damage. The IRDAI Information and Cyber Security Guidelines therefore establish cybersecurity as an enterprise governance, operational-resilience and policyholder-protection requirement—not merely an IT compliance activity.
Independent information security assurance audits help insurers and applicable insurance intermediaries demonstrate whether required controls are appropriately designed, implemented and operating effectively. The audit is significant to the insurance sector because it supports:
- Board and senior-management oversight of cyber risk and regulatory compliance.
- Protection of policyholder, financial, health and other sensitive information.
- Identification of control weaknesses before they result in material incidents.
- Resilience of critical insurance services, applications and digital channels.
- Oversight of cloud, outsourcing, technology-vendor and intermediary risks.
- Evidence-based regulatory reporting, remediation and compliance monitoring.
Codec Networks’ IRDAI Information and Cyber Security Assurance Audit service is relevant to insurers, insurance intermediaries and other applicable regulated entities seeking an independent and structured assessment against the IRDAI-prescribed control framework. Codec Networks follows a structured, independent and evidence-based audit methodology aligned with the CERT-In Comprehensive Cyber Security Audit Policy Guidelines and specifically adapted to the IRDAI Information and Cyber Security Guidelines, 2026. The methodology evaluates applicable controls across governance, technology, people, processes, cloud services, third parties, incident response and cyber resilience.
The audit is risk-based: regulatory significance, business criticality, data sensitivity, cyber exposure, previous incidents, technology changes and third-party dependencies determine the depth of testing and sample sizes. Risk-based prioritization does not exclude any mandatory IRDAI requirement from the applicable audit scope.
Our Eight-Stage Audit Methodology
1. Independence and Engagement Acceptance
Codec Networks verifies auditor independence, conflicts of interest, team competence, confidentiality, secure evidence handling and engagement eligibility. The audit scope, review period, locations, deliverables, authorized contacts and testing restrictions are documented before commencement.
2. Applicability and Scope Determination
The auditee is classified according to its IRDAI-regulated category. Applicable requirements are mapped using IRDAI Section 1.4, Circular Paragraph 5 and Annexures I and II. The audit universe includes applicable business services, locations, information assets, applications, infrastructure, cloud environments and critical third parties.
Any exclusion or “Not Applicable” classification must be supported by documented justification and auditee confirmation.
3. Risk Profiling and Audit Planning
Audit areas are prioritised based on:
- Regulatory and policyholder impact.
- Criticality of business services and information assets.
- Sensitivity and volume of information processed.
- Internet exposure and privileged-access risks.
- Previous incidents, findings and overdue actions.
- Cloud, outsourcing and vendor concentration.
- Significant system, process or organizational changes.
Higher-risk areas receive expanded sampling, specialist review and deeper technical validation.
4. Evidence Collection and Control Walkthroughs
Codec Networks issues a structured evidence request and conducts walkthroughs with business, IT, information-security, risk, compliance and vendor-management teams. Evidence may include policies, inventories, contracts, configurations, logs, tickets, approvals, committee records, incident reports, VAPT reports and business-continuity test results.
Audit conclusions are based on relevant, reliable and verifiable evidence. Management assertions, interviews or screenshots are corroborated before being relied upon.
5. Test of Design and Operating Effectiveness
Each applicable control within the prescribed IRDAI audit checklist is evaluated separately for:
- Test of Design: Whether the control is appropriately defined, approved, owned, monitored and capable of meeting the regulatory objective.
- Test of Operating Effectiveness: Whether the control operated consistently and effectively throughout the audit period.
Testing may include inspection, observation, reperformance, population analysis, representative sampling and technical corroboration.
6. Authorized Technical Validation
Where included in the approved scope, Codec Networks validates application, infrastructure, cloud, network, access, logging, vulnerability-management, backup, restoration and security-monitoring controls. Intrusive testing is undertaken only with written authorization and agreed Rules of Engagement.
7. Risk Rating and Reporting
Audit observations are supported by objective evidence and rated using the applicable IRDAI Annexure III methodology. Ratings consider regulatory significance, likelihood, business and policyholder impact, exposure, affected population, duration, recurrence and compensating controls.
Deliverables may include:
- Executive and Board-level summary.
- Applicability and control-mapping statement.
- Completed Annexure III audit checklist.
- Risk-rated findings with evidence references.
- Management action and remediation plan.
- Applicable Annexure V or VI certificate.
- Revalidation and closure report.
8. Remediation Review and Closure
Management responses, corrective actions, owners and target dates are documented without affecting Codec Networks’ independent audit judgement. Completed corrective actions are revalidated w.r.t. current and representative evidence before being reported as closed. Open, partially remediated or risk-accepted findings are transparently reported with their residual risk.
Please Note
The audit is an independent, point-in-time assessment performed as per the agreed scope, applicable criteria and evidence made available during the engagement. Audit conclusions may be affected by incomplete inventories, unavailable evidence, scope limitations, system changes, third-party restrictions or management representations. The auditee remains responsible for determining its legal and regulatory obligations, maintaining effective controls, remediating findings, accepting residual risk and making submissions to IRDAI.
Codec Networks applies an independent, risk-based and evidence-driven audit approach aligned with the CERT-In Comprehensive Cyber Security Audit Policy Guidelines and tailored to the IRDAI Information and Cyber Security Guidelines, 2026. The approach is designed to assess whether applicable cybersecurity controls are appropriately governed, adequately designed and operating effectively across the auditee’s business, technology and third-party environment.
Risk determines the depth of testing, sampling and specialist involvement; it does not remove mandatory IRDAI requirements from the applicable audit scope.
Key Principles of Our Audit Approach
Regulatory Applicability First
We begin by identifying the auditee’s regulated category, applicable IRDAI requirements and relevant control universe. Business services, information assets, applications, infrastructure, cloud services, locations and critical third parties are mapped to the audit scope.
Independence and Professional Judgement
Auditors are assigned after evaluating competence, independence, conflicts of interest and confidentiality obligations. Findings and risk ratings are determined from evidence and professional judgement and are not influenced by commercial outcomes, management expectations or remediation status.
Business and Policyholder Risk Focus
The audit considers the potential impact of control failures on critical insurance services, policyholder information, claims, payments, regulatory obligations, service availability and organizational reputation. Higher-risk services and assets receive deeper testing and expanded sampling.
Evidence Over Assertion
Management interviews and representations are corroborated through policies, records, logs, configurations, tickets, approvals, contracts, system-generated reports and other verifiable evidence. Every material conclusion is traceable to an applicable criterion, audit procedure and supporting evidence.
Separate Evaluation of Design and Effectiveness
Codec Networks distinguishes between:
- Control design: Whether the control is properly defined, approved, owned and capable of meeting its intended objective.
- Operating effectiveness: Whether the control operated consistently and effectively during the audit period.
This distinction enables accurate identification of policy, governance, implementation and operational failures.
Integrated Governance and Technical Review
The audit combines governance and process assessment with authorized technical validation. Depending on scope, the review may cover access controls, configurations, applications, infrastructure, cloud environments, vulnerability management, logging, SOC operations, incident response, backup, restoration, business continuity and third-party security.
VAPT and intrusive testing are undertaken only where specifically authorized under written Rules of Engagement.
Transparent Risk Rating and Reporting
Observations are assessed according to regulatory significance, likelihood, business and policyholder impact, exposure, affected population, recurrence, duration and compensating controls. Findings are supported by evidence and presented with practical remediation outcomes, responsible owners and target dates.
Deliverables may include:
- Executive and Board-level summary.
- Applicability and scope statement.
- Completed Annexure III checklist.
- Risk-rated audit observations.
- Evidence and compliance index.
- Management action plan.
- Applicable Annexure V or VI certificate.
- Remediation-revalidation report.
Closure and Continuous Improvement
Corrective actions are independently revalidated before findings are reported as closed. Partially implemented, overdue or risk-accepted observations remain transparently reported with their residual risk and governance status. The approach supports sustainable cybersecurity improvement beyond periodic checklist compliance.
Please Note:
The audit is an independent, point-in-time assessment of the agreed scope and evidence made available. The auditee remains responsible for regulatory compliance, effective control operation, remediation, risk acceptance and required submissions.

FREQUENTLY ASKED QUESTION
Asking the right questions is the first step toward security; our FAQs
The Guidelines apply to all insurers, including Foreign Reinsurance Branches, and specified insurance intermediaries such as brokers, corporate agents, web aggregators, TPAs, IMFs, insurance repositories, ISNP operators, corporate surveyors, MISPs, CSCs and the Insurance Information Bureau of India. Exact applicability depends on the entity category and Annexures I and II. Individual agents, POSPs and individual surveyors are not directly covered in the same manner, but insurers must oversee their minimum security requirements.
Yes. Applicable regulated entities are required to undergo an annual independent information and cybersecurity assurance audit in accordance with IRDAI Section 1.10. Audit timing, report format, governance review and submission requirements vary according to the type of regulated entity.
The auditor must satisfy the applicable eligibility and independence requirements prescribed by IRDAI, including Annexure IV, and relevant CERT-In requirements. The auditee should verify the auditing organization’s current empanelment status, approved capabilities, declared resources, independence and relevant insurance-sector experience before appointment.
Not necessarily. The applicable control universe depends on the regulated entity’s classification and Annexures I and II. Any control marked “Not Applicable” should have a documented and supportable rationale; mandatory applicable controls cannot be excluded merely because a system is outsourced, cloud-hosted, group-managed or operated by a third party.
Insurance intermediaries are generally required to provide the applicable audit report and governance comments to their insurer or insurers within 30 days of audit completion. Insurers are generally required to submit the signed report and Board comments to IRDAI within 90 days from the end of the financial year or 30 days from audit completion, whichever is earlier. The latest IRDAI directions applicable on the submission date should always be checked.
The audit may cover cybersecurity governance, risk management, information assets, identity and access management, data protection, application security, infrastructure, cloud services, security monitoring, incident response, business continuity, disaster recovery, third-party security, privacy and regulatory reporting. Final coverage is based on the auditee’s applicable control universe and approved engagement scope.
Typical evidence includes policies, procedures, asset and vendor inventories, risk registers, committee minutes, access records, configurations, logs, vulnerability and patch records, incident reports, VAPT reports, backup and restoration results, BCP/DR exercise records, contracts, SLAs and regulatory submissions. Evidence should be current, complete, traceable to its source and available through approved secure channels.
Codec Networks considers regulatory significance, business criticality, policyholder impact, information sensitivity, cyber exposure, previous incidents, control failures, technology changes and third-party dependencies. Higher-risk areas receive deeper testing and larger samples, but risk-based prioritisation does not remove mandatory IRDAI controls from the applicable audit scope.
VAPT is a distinct technical assessment with separate scope, frequency, authorization and reporting requirements. Where included in the engagement, it is conducted under written Rules of Engagement. Existing VAPT reports may be reviewed as audit evidence, but their scope, environment, methodology, auditor eligibility, findings and remediation status must be independently validated.
Audit information is handled under agreed confidentiality, access, retention, secure-transmission and disposal requirements. Reports and evidence are shared only with authorized recipients, subject to applicable legal, regulatory and CERT-In disclosure obligations. Technical testing is performed only after written authorization and within agreed safety restrictions.
Depending on the agreed scope, deliverables may include an applicability memorandum, risk-based audit plan, evidence index, completed Annexure III checklist, detailed audit report, executive or Board summary, management action plan, applicable Annexure V or VI certificate and remediation-revalidation report.
Findings are evaluated using the applicable IRDAI Annexure III design-and-effectiveness methodology. The rating considers regulatory significance, likelihood, business and policyholder impact, exposure, affected population, duration, recurrence and available compensating controls. Each reportable observation is supported by objective evidence and a documented rating rationale.
No. An audit certificate is issued only when supported by sufficient and appropriate evidence and subject to the prescribed certificate conditions. Scope limitations, unresolved findings, unavailable evidence or material control failures may result in qualifications, limitations or non-issuance of an unqualified certificate.
The auditee organization remains responsible for corrective actions, target dates, interim safeguards and residual-risk acceptance. Codec Networks may recommend required security outcomes and subsequently revalidate completed actions, but closure is reported only when implementation and operating effectiveness are supported by satisfactory evidence.
No. The audit is an independent, point-in-time assessment of the agreed scope and evidence made available. It does not constitute legal advice, IRDAI or CERT-In certification, endorsement or guaranteed regulatory acceptance, and it cannot guarantee identification of every vulnerability or prevention of future incidents. Continued compliance remains the responsibility of the auditee organization.
